About the job
Sierra is building a platform to enable companies to create better, more human customer experiences with AI. Our healthcare agents are transforming patient interactions by handling sensitive conversations, from explaining benefits to resolving billing questions with empathy. We partner with leading health plans, providers, and healthcare platforms to deliver AI agents that make care more accessible, human, and affordable. Join us to transform the patient experience across the healthcare system.
Responsibilities
- Design and build core healthcare integration primitives for safe connection with EHRs and other healthcare systems.
- Own production integrations with systems like Epic, athenahealth, Oracle Health, eClinicalWorks, MEDITECH, using FHIR, HL7, APIs, flat files, and custom patterns.
- Build reusable platform capabilities for healthcare data access, write-back workflows, authentication, authorization, auditability, observability, testing, and deployment.
- Partner with Agent Engineers to design and ship healthcare workflows such as appointment scheduling, patient intake, and care navigation.
- Serve as a technical SME with customers' EHR, IT, security, and operations teams, translating requirements into implementation plans.
- Define standards for safe healthcare agent actions, including validation, idempotency, permissions, human-in-the-loop review, escalation, and PHI/data privacy safeguards.
Requirements
- 8+ years of hands-on software engineering experience, operating at or above Senior Software Engineer level.
- Deep experience shipping production healthcare integrations under HIPAA, security, privacy, and compliance constraints.
- Strong technical fluency with healthcare interoperability standards and patterns (FHIR, HL7, OAuth/SMART, patient identity, data mapping, audit logs, EHR-specific APIs).
- Experience building integrations with major EHRs or healthcare platforms (Epic, athenahealth, Oracle Health, eClinicalWorks, MEDITECH, or similar).
- Strong product and platform judgment for designing reusable primitives and abstractions.
- Comfort working directly with customers (EHR analysts, security teams, executives) while remaining hands-on in code.
- Degree in Computer Science or related field, or equivalent professional experience.
